Youth Art Program Registration

Program Overview: The PFA Youth Art Program is designed to provide a creative outlet for youth while simultaneously offering support for their guardians. The program includes art sessions for youth and concurrent support meetings for guardians. A light meal will be served during each session. Artwork created by the youth may be displayed in the PFA office, and updates on the program will be provided as needed.

Policies:

  1. Attendance:
    • Youth participants must be brought to the meetings by an adult guardian.
    • The adult guardian is required to attend the support meeting taking place at the same time as the youth art session.
  2. Supervision:
    • Youth participants must remain under the supervision of program facilitators during the art sessions.
    • Guardians must remain on the premises for the duration of the session.
  3. Meals:
    • A light meal will be provided for both youth participants and their guardians.
  4. Artwork Display:
    • Artwork created by youth participants may be displayed in the PFA office.
    • Guardians and participants will be notified in advance if their artwork is selected for display.
  5. Updates:
    • Quarterly updates on the program’s progress will be provided to guardians whose youth attended the previous quarter.

Disclaimers & Safety Information:

  1. Health and Safety:
    • If your child has any allergies, medical conditions, or special needs, please notify the program facilitators in advance to ensure proper care and accommodation.
    • Youth participants are expected to behave respectfully towards facilitators, peers, and the property.
    • Disruptive or harmful behavior may result in removal from the program
  2. Liability:
    • PFA is not responsible for any injuries or accidents that may occur during the program. Guardians are required to sign a liability waiver acknowledging this.
    • Guardians are responsible for their youth before and after the scheduled program times.
  3. Photography and Media:
    • By participating in the program, you consent to your child’s image being used in PFA promotional materials, including social media, unless otherwise specified.

Agreement:

By completing the youth registration form below, you acknowledge that you have read and agree to the policies and disclaimers outlined above for the PFA Youth Art Program.
